The Healthy Regions InterVEntion (THRIVE) aims to find new ways to prevent and manage chronic disease. We want to help people be healthier through personalised coaching, easy-to-follow advice and advanced health technology. We are currently operating in rural and regional Victoria.
Chronic disease is a serious problem. It is responsible for around 90% of the total disease burden in Australia. This burden is even higher in rural and regional communities.
Chronic conditions such as heart disease and diabetes make day-to-day life more challenging, and put people at greater risk of future health problems. They are also costly to our health system and to society. But chronic disease is preventable and treatable.
For people with a chronic condition, simple lifestyle changes can improve health and wellbeing, and reduce the risk of future problems. Lifestyle changes can also help prevent chronic disease from occurring in the first place.
We want to help you find effective ways to make these lifestyle changes.
THRIVE is delivered by a dedicated team located at Federation University and the University of Melbourne in collaboration with colleagues in the United Kingdom and United States and a range of clinical agencies and digital health providers.
