Be part of a proven initiative to reduce the burden of chronic disease in your patient community.
At an initial two-day event, you will be trained in delivering the health intervention component of THRIVE.
Training will involve:
You will be asked to support the THRIVE team in recruiting patients to the program. This will involve signposting patients to an online survey.
We will equip you to answer questions patients have about THRIVE.
After you have completed the training, you will be linked up with patients in your local area in order to deliver the health intervention component of THRIVE. You will meet each person once a month for a coaching session. The time you spend with patients will be part of your regular practice.
If you have been trained in using the THRIVE digital health platform, you will have the chance to use the app as part of the coaching you deliver.
You will be asked by the THRIVE team to collect and feed back data as part of the quantitative evaluation of the project. Feedback should take less than two hours of your time.
You may also be invited to take part in a more in-depth discussion about your experience delivering THRIVE. This would also take about two hours of your time.
The THRIVE team will be available to support you with the lifestyle changes program and digital health platform.
